Adult Community Support Services

Community Support Services (CSS) is an variety of supports and services to help adults who have a severe and persistent mental illness (SPMI). This program provides effective integrated and coordinated treatment right in their community to assist with mental health conditions such as major depression, bipolar disorder or schizophrenia. Their condition is serious and may have resulted in a hospitalization. These services are proven to increase ability to live independently, develop skills to maintain employment, build social relationships and a sense of community and prevent the need for hospitalization. These services are designed to be convenient for those we serve by offering support in their home, community, and even their place of employment.

Each individual drives their own treatment, expressing their preferences, focusing on their strengths, and working with their treatment team to develop an individualized treatment plan intended to get the best results.

Case Management

Case management is a medically necessary, one-on-one service. Most case management services are provided in the person’s home or community. Case managers coordinate care among treatment providers and follow the medically necessary treatment plan. Some goals a case manager may work on with an individual include symptom reduction, building coping skills, promoting wellness, developing healthy relationships, and strengthening independent living skills such as financial management, nutrition, self-care, and recreation.

Attendant Care

Attendant care is an additional service that provides friendly support for everyday functioning. Attendant care services enable individuals to accomplish tasks or engage in activities they would normally perform themselves if they did not have behavioral health challenges. Examples of assistance may include supervision to maintain safety, ensuring that basic needs are met and promoting independence.

Psychosocial Groups

Participants experience social and emotional support and develop skills to be successful in home and community settings. These specialized groups develop intrapersonal skills, self-regulation of emotions and cover many topic areas throughout the process of recovery. Groups also assist in having individuals develop a sense of community and belonging.

Certified Peer Support

Certified Peer Support is a person-centered service with a focus on restoring the individual to their previous level of functioning. Services can be one-on-one or in a group. It is designed to help with managing and coping with psychiatric or substance use symptoms using community resources. Providers are self-identified recipients of behavioral health services. Peer support emphasizes the importance of supporting one another and taking full advantage of “lived experiences” as valuable resources to be used on the road to recovery.

Individual Placement Support

Job readiness training programs prepare individuals to get, keep and excel at their job. Basic employability skills include effective communication, problem solving, resume building, and interviewing. Our program also promotes good work habits that facilitate their ongoing success. Individuals will develop interpersonal communication skills, problem solving and personal management skills.

Housing Support

Case Managers work with individuals diagnosed as SPMI on obtaining housing through the variety of options available locally. A limited amount of supported housing funds are available through the State of Kansas, with priority usage for re-housing homeless individuals or preventing homelessness.

Benefits Counseling

Benefits counseling assists eligible individuals in applying for disability, low cost health coverage, SSI (Supplemental Security Income), or other benefits. Central Kansas Mental Health Center partners with the Kansas Department of Health and Environment to provide SOAR* Trained Case Managers to streamline the process and obtain needed resources.

*SOAR stands for: SSI/SSDI, Outreach, Access, Recovery
